How do I request my medical records from Advocare?

The easiest and most convenient way to access your medical records is to request them online through Patient Request. Click below to submit a request to access your records anytime, anywhere. Please have your driver’s license or other government ID ready.
To request your medical records in person, please complete the “Authorization For Use/Disclosure of Protected Health Information” form, and return the form to your Care Center so that they can process your request.
