Advocare Primary Care Offices Achieve PCMH

Advocare Primary Care Practices received NCQA Patient-Centered Medical Home Recognition. NCQA is a private, non-profit organization dedicated to improving health care quality. The Recognition Programs are built on evidence-based, nationally recognized clinical standards of care and they were developed to help practices support the delivery of high-quality care.
